1. File a Police Report

  • Call your local police agency and file a police report
  • Provide as much detail as possible
  • Obtain a copy of the police report as soon as possible

TRACK ALL COMMUNICATIONS THROUGHOUT THE PROCESS

2. Protect your Credit

  • Place Initial Fraud Alert  by                                               - Calling ONE of the three credit reporting companies                                        OR             Sending a letter to ONE of the reporting companies and     ask them to put an initial fraud alert on your credit report.                                                                                                    Whichever company you contact is responsible for    alerting the other two credit reporting companies.                                                                                                   
  • Place a Credit Freeze by                                                   - Calling ALL three credit reporting companies                                                  OR         Sending a letter to ALL of the reporting companies and     ask them to put a credit freeze on your credit report.

3. Create an Identity Theft Report

Your Identity Theft Affidavit will assist you during your response & recovery process. You will attach it to your correspondence to various agencies for the purpose of

  • Having fraudulent information removed
  • Stopping debt collectors from attempting to collect fraudulent debts or reporting the fraudulent debt to debt collectors
  • To acquire information you need to clear your name

The Identity Theft Affidavit and Police Report together create your Identity Theft Report. This combined information will provide details needed to clear your name.

4. Order your Credt Reports

Contact EACH credit reporting company to obtain your free credit report. Ask to have only the last 4 digits of your social security number listed on the report.

 

You will need to provide:

  • Full name, former names, suffixes
  • Current address, and any addresses for the last 5 years
  • Copy of a document with proof of your social security number- social security card, paystub, w2
  • Proof of residence-utility bill, creditors statement

STEP TWO

1. Request Copies of Documents the Identity Thief used to open Accounts

The sample letter to the right will assist you in acquiring documents/business records that the identity thief used to open fraudulent accounts or make fraudulent charges in your name.

STEP THREE

1. Blocking a Credit Card Company from Reporting Incorrect Information to the Credit Reporting Companies

Credit card companies report when you are delinquent with your payments to the 3 credit bureaus. (Equifax, Experian, TransUnion). These 3 companies compile all of your credit activity over your lifetime.
 

The sample letter to the right will assist you in communicating to the credit card company that

  • you are not the holder of their credit card
  • you are a victim of identity theft
  • you did not make the transactions which they are reporting as delinquent to the credit bureau
  • they should cease reporting the delinquent charges

2. Blocking the Credit Reporting Company (aka Credit Bureau) from Reporting Incorrect Information on your Credit Report

  • Identify item(s) to be blocked by name of the source, such as credit card issuer or bank
  • Identify type of item, such as credit account, checking account, etc.

Enclosure Section: describe any enclosed documentation, such as payment records, court documents supporting your position.
