AGENCY DESIGNATION IN INDIAN RIVER, MARTIN, OKEECHOBEE, AND ST. LUCIE COUNTIES AND PALM
BEACH COUNTY SERVICE AREAS A AND B
The Area Agency on Aging of Palm Beach/Treasure Coast, Inc. is requesting proposals from organizations that have the operational capacity and interest in being designated as the Lead Agency for the Community Care System for each of the Community Care Service Areas (CCSAs) in Planning and Service Area 9 beginning July 1, 2021. The CCSAs include Indian River, Martin, Okeechobee, and St. Lucie counties as well as Palm Beach County Service Area A and Palm Beach County Service Area B. Separate proposals are being requested for each CCSA for the provision of services to all eligible CCE, HCE, and ADI clients in the CCSA. Organizations interested in obtaining Community Care System Lead Agency designation are requested to submit written proposals. An original, six (6) copies and one (1) electronic copy (PDF format preferred) on a CD or flash drive must be submitted in sealed envelopes/packages addressed to Area Agency on Aging of Palm Beach/Treasure Coast, Inc., 4400 North Congress Avenue, West Palm Beach, FL 33407.
